Services Provided
What we offer
Management Services Organization (MSO) Structure: Impact Health LLC DBA Impact Health Clinics ("Impact Health") is a management services organization that provides administrative, operational, and management services to Impacting Lives LLC, a Mississippi professional entity. Impact Health does not provide medical care, make medical decisions, or practice medicine.
Medical Services: All medical services, clinical care, and treatment decisions are provided exclusively by Impacting Lives LLC and its licensed healthcare providers. Impacting Lives LLC maintains exclusive control over all clinical decisions, medical protocols, patient care, and medical records.
Management Services Agreement: Impact Health and Impacting Lives LLC operate under a Management Services Agreement (MSA) that clearly separates management services from medical services, ensuring compliance with Mississippi corporate practice of medicine laws.
Services Available Through Impacting Lives LLC
Through our management services arrangement with Impacting Lives LLC, the following medical services are available:
- Testosterone Replacement Therapy (TRT)
- Medical Weight Loss Programs
- Advanced Peptide Therapies
- NAD+ Therapy
- Comprehensive Health Assessments
Service Limitations
Our services are provided for informational and treatment purposes only. We do not:
- Provide emergency medical services
- Replace primary care physician relationships
- Guarantee specific treatment outcomes
- Provide services outside our scope of practice
SMS/Text Messaging Terms
Terms for our text messaging program
Program Description
Impact Health Clinics offers an optional SMS/text messaging program to provide appointment reminders, health information, lab results, and other healthcare-related communications. By providing your mobile phone number and opting in, you consent to receive automated text messages from Impact Health Clinics.
Consent and Opt-In
By opting in to receive text messages, you:
- Expressly consent to receive automated text messages at the mobile number provided
- Confirm that you are the subscriber or authorized user of the mobile number
- Understand that consent is not a condition of purchase or receiving care
- Acknowledge that message and data rates may apply based on your carrier plan
- Agree to notify us immediately if your mobile number changes
Message Frequency and Content
Message frequency varies based on your treatment plan and preferences. You may receive:
- Up to 4-6 messages per month for routine communications
- Additional messages during active treatment periods
- Urgent messages regarding test results or health concerns as needed
- Administrative messages regarding appointments or account issues

Carrier and Device Compatibility
Our SMS program is compatible with most major U.S. carriers including AT&T, T-Mobile, Verizon, Sprint, and other participating carriers. We are not liable for:
- Messages not delivered due to carrier limitations or outages
- Delays in message delivery
- Message and data charges from your carrier
- Incompatibility with certain devices or carriers
Privacy and Security
We take the privacy and security of your information seriously. Text messages may contain protected health information (PHI) and are transmitted securely. Payment Terms and Processing
How we handle payments
Payment Processing
We use Stripe, Inc. ("Stripe") as our payment processor to securely process all payment transactions. By making a payment through our services, you agree to Stripe's Terms of Service, available at https://stripe.com/legal.
When you provide payment information, it is securely transmitted to Stripe for processing. We do not store your full credit card numbers or sensitive payment data on our servers. All payment data is encrypted and handled according to PCI-DSS Level 1 security standards.
Payment Methods
We accept the following payment methods:
- Credit cards (Visa, Mastercard, American Express, Discover)
- Debit cards
- ACH bank transfers (where available)
- Other payment methods as may be offered through Stripe
Payment Authorization
By providing payment information, you authorize us to charge the payment method you provide for:
- Consultation fees and service charges
- Treatment costs and medication fees
- Recurring subscription or membership fees (if applicable)
- Any outstanding balances or late fees
Refund Policy
Refund policies vary by service type:
- Consultation Fees: Consultation fees are generally non-refundable but may be applied toward your first month of treatment if you proceed with services.
- Treatment Services: Refunds for treatment services are evaluated on a case-by-case basis. Contact us within 30 days of service to discuss refund eligibility.
- Medications and Products: Due to the nature of prescription medications and medical products, refunds may be limited. Unopened, unexpired products may be eligible for return within 30 days of purchase.
- Processing Time: Approved refunds will be processed within 5-10 business days and will be credited to the original payment method.

Failed Payments and Declined Transactions
If a payment is declined or fails:
- We will attempt to notify you of the failed payment
- You are responsible for updating your payment information or providing an alternative payment method
- Services may be suspended until payment is successfully processed
- We are not responsible for fees charged by your financial institution for declined transactions
Dispute Resolution for Payments
If you have concerns about a charge or payment:

- You may also contact your financial institution to dispute a charge

Payment Processing Liability
We are not responsible for errors or delays in payment processing caused by third-party payment processors, financial institutions, or technical issues beyond our control. Stripe is responsible for the security and processing of payment transactions. We are not liable for any losses, damages, or unauthorized transactions that occur due to issues with Stripe's services or your payment method.

You are responsible for providing accurate and complete information about your:
- Medical history and current health conditions
- Current medications and supplements
- Allergies and adverse reactions
- Contact information and insurance details
Treatment Compliance
You agree to:
- Follow prescribed treatment plans
- Attend scheduled appointments
- Take medications as directed
- Report any side effects or concerns promptly
- Participate in follow-up care and monitoring
Payment Obligations
You are responsible for:
- Payment of all fees and charges
- Understanding your insurance coverage
- Providing updated insurance information
- Paying copays and deductibles as required
